Illinois requires certain health records before a child can start school. Get them in early — students without the required records are excluded beginning October 15.
Entering kindergarten
A physical exam and up-to-date immunizations are due before the first day of school.
Immunizations:
- Polio — 4 or more doses, the last on or after the 4th birthday
- DTP / DTaP — 4 or more doses, the last on or after the 4th birthday
- MMR — 2 doses, on or after the 1st birthday
- Varicella (chickenpox) — 2 doses, on or after the 1st birthday
The physical must also include a diabetes screening and a lead-risk questionnaire.
Eye exam — due before school starts (report cards are held if not received by October 15). Dental exam — due before school starts (report cards are held if not received by May 15).
Entering 6th grade
Under Illinois School Code (Health Law, Section 665):
Immunizations:
- Tdap — 1 dose
- Meningococcal — 1 dose
- Hepatitis B — 3 doses
- MMR — 2 doses
- Varicella — 2 doses
- Polio — proof of vaccination
A physical exam completed within 12 months of enrollment is due by October 15. It is valid for 13 months and also serves as the sports physical. A dental exam is due by May 15.
